DAG design for site loss

Hello,

We have two sites.  Site A and Site B.  We have a DAG setup with member at A and a member at B.  Witness server at site A.   We recently had a full site down at Site A and our databases did not activate at site B.  Is this because we lost the witness at the same time?  Should we have witness server at a third site...?
