Witness in failed state

More often than not, when I try to start dag maintenance, I get this message

WARNING: Database availability group ‘dag-mydag’ witness is in a failed state. The database availability group requires the witness server to maintain quorum. Please use the Set-DatabaseAvailabilityGroup cmdlet to re-create the witness server and directory.

My witness server is one of my Hub Transport servers. If I go to the GUI, and I just retype the witness server name, and then run StartDagMaint again....I do not get the error.

Why does this keep happening and how do I correct permanently?

Also, last night, my database failed over, and in event viewer, it says the it lost quorum.....the hub transport server is always up, and I never have problems connecting to it.

I'm running Exchange 2010 SP3 Rollup 4, on Windows Server 2012.
